Each of the winners must complete and return an affidavit of eligibility, liability and publicity release within seven days of receiving the notification. Hence, if a prize winner cannot be reached for three business days or fails to submit the above affidavit in time, he or she will forfeit the prize and a different winner will be selected from other eligible entries.
